We are seeking an experienced Hospice or Home Health Liaison in Sarasota, FL.

This is a full-time, permanent, salaried position with a trusted senior living operator. Our client places employees at the forefront. You can expect a salary of $75,000 to $85,000 per year depending on experience. This role is hiring immediately.

Qualifications

  • OPTIONAL state RN, LVN, or LPN license
  • 2+ years of nursing experience in long-term care, LTAC, skilled nursing, hospice, home health, palliative care, or RN clinic environment
  • Prior experience as a Registered Nurse, Licensed Vocational Nurse, Licensed Practical Nurse, Travel Nurse, or Clinical Liaison
  • Excellent skills in communication, management, organization, and problem-solving
  • Knowledge of nonprofit organizations, fundraising, and other relevant skills are a plus

Responsibilities

  • Ensuring high-quality care that is compliant with all applicable laws, regulations, and standards
  • Evaluating patient needs and assessing necessary level of care — including medication and other supports
  • Coordinating with hospital, family, and physicians
  • Ensuring seamless services as patients transition to at-home hospice care
